My bid was the same as the 'winner' - why did they get the item and I didn't?

You can't win the item for the same price, your bid must be higher (at least, at some certain increment) to win.
That's how eBay works.

But if the bids were the same value, how does ebay determine who 'wins' the item?

The earliest bid wins in this case.
